But there’s a lot of confusion around it: 2% or 5%, Rogaine or Alopexy, how long to wait, and what happens when you stop? Here’s a simple guide based on official patient information leaflets and studies, so you can discuss it with your doctor with a clear understanding of what it involves.

I’m not a doctor. This guide isn’t a substitute for advice from your dermatologist or pharmacist. If you’re looking for my personal experience after almost 10 years of use, you’ll find it in my A personal account of using minoxidil.

The essentials in 6 points

In France, 2% minoxidil is the only strength indicated for women.

It helps some women, but only to a modest extent: in the largest review of studies, 26% of women saw regrowth, compared with 14% using a product with no active ingredient (a placebo).

Allow about 4 months or more before assessing the results.

It works for as long as you apply it: if you stop, you’ll return to your starting point within 3 or 4 months.

It’s not recommended during pregnancy and must not be used while breastfeeding.

Avoid all contact with babies, and be very careful if you have a cat: it’s highly toxic to them.

What is minoxidil?

It’s a medication available over the counter at pharmacies as a lotion applied to the scalp. It’s indicated for moderate hair loss related to androgenic alopecia (also called androgenetic alopecia). Exactly how it works isn’t known; the official patient information leaflet says so itself.

Does minoxidil work for women?

For some women, yes, but usually only to a modest extent. The largest review of studies on the subject (Cochrane, 47 studies and over 5,000 women) reports these figures: 26% of women who used minoxidil noticed regrowth, compared with 14% using a product with no active ingredient. This means minoxidil is almost twice as effective, but also that many women don’t see regrowth.

According to the patient information leaflet, results appear to be better in younger people and when hair loss is recent or limited. There’s no way to know in advance whether it will work for you.

Minoxidil 2% or 5%: Which should women use?

In France, the patient information leaflet indicates 2% minoxidil for adults, both men and women. 5% minoxidil is indicated for men, and its leaflet advises women against using it: it cites low effectiveness in women and unwanted hair growth outside the scalp in 37% of cases.

What about the studies? Across four studies involving 1,006 women, 5% was no more effective than 2%. Some dermatologists still prescribe 5% for women: this is a medical decision to make with your doctor.

Rogaine, Regaine, Alopexy: are they the same?

Yes, these are brand names for the same medication: minoxidil. Rogaine is the name used in the United States. Regaine is the name used in several European countries, such as Spain, Portugal, and Germany. In France, you’ll find Alopexy, Alostil, or Bailleul. At the same dosage, the active ingredient is the same; only the lotion’s ingredients and format differ.

Please note: recommendations vary by country. In the United States, there is a 5% Rogaine foam for women. In Austria and Germany, there is also a 5% foam for women, applied once a day. That’s why you’ll find conflicting advice online: an American or German user may not be talking about the same product as the one described in the French patient information leaflet. Before ordering a product from abroad, ask your pharmacist for advice.

How do you apply minoxidil?

For 2% minoxidil, the patient information leaflet says:

1 ml in the morning and 1 ml in the evening, starting at the center of the area being treated. The dose is the same regardless of the size of the area.

Do not use more than 2 ml per day: using more may increase absorption through the skin.

Apply to completely dry hair and scalp.

Wash your hands thoroughly before and after, and spread the product with your fingertips.

Never apply it to any other part of the body, or to a red, irritated, or sore scalp.

The product is flammable, and an opened bottle can be kept for 1 month.

If you wear a hair topper or a wig, good news: according to the patient information leaflet, this does not affect how much of the product is absorbed. Just let the lotion dry before putting it on.

How long does it take to see results?

Patience is key: the first effects appear after about 4 months of use, sometimes longer, and results vary considerably from person to person. One specialist review even recommends waiting 12 months before assessing the results. For an honest comparison, take photos in the same lighting and location at regular intervals.

Why do you lose more hair at the start of treatment?

This is known as “shedding.” According to the patient information leaflet for 5% minoxidil, hair loss may temporarily increase during the first 2 to 6 weeks: hairs that were about to fall out shed sooner. If hair loss continues for more than 6 weeks, the leaflet says to stop using the product and consult a doctor.

What are the side effects of minoxidil?

According to the patient information leaflet, these are mainly minor skin reactions. Headaches are very common. Other common side effects include hair growth on parts of the body other than the scalp, itching, irritation, dry or red skin at the application site, acne, muscle pain, swelling, shortness of breath, and depression.

Stop using the product and see a doctor if you experience low blood pressure, chest pain, a very rapid heartbeat, faintness or dizziness, sudden unexplained weight gain, swelling in your hands or feet, or persistent redness of the scalp. If you have heart disease or a heart rhythm disorder, ask a doctor for advice before starting.

To limit facial hair growth, wash your hands thoroughly and rinse off any product that has run onto your forehead or temples. This hair usually disappears within 4 months of stopping treatment.

Minoxidil, pregnancy, and breastfeeding

Minoxidil 2% is not recommended during pregnancy or for women of childbearing age who are not using contraception. It must not be used while breastfeeding, as it passes into breast milk. The Cochrane review also advises against using it during pregnancy or while breastfeeding.

It is also not recommended for anyone under 18 or over 65, due to a lack of data. It is also not indicated if there is no family history of hair loss, or in cases of sudden or patchy hair loss, hair loss after childbirth, or when the cause is unknown. That’s why it’s important to have a doctor confirm the diagnosis before starting.

Babies, children, and pets: precautions to know

Babies. Cases of hair growing on infants’ bodies have been reported following contact with the skin of an adult using minoxidil. This went away within a few months once contact stopped. The European Medicines Agency had this warning added to the package inserts: don’t let your children touch the areas where you’ve applied the product.

Children. Keep the bottle out of their reach: if accidentally ingested, it can have serious effects on the heart.

Cats. Minoxidil is highly toxic to them. Just a few drops may be enough—for example, if they lick your head or pillowcase. In the cases studied by the American Academy of Dermatology, almost all the animals were hospitalized and 10 out of 68 cats died. If your cat may have been exposed, call your veterinarian right away.

What happens when you stop using minoxidil?

Minoxidil doesn’t cure alopecia: it works only as long as you apply it. When you stop, regrowth ceases and you return to your starting point in 3 or 4 months, according to the package insert. That’s why you should think of it as a long-term commitment and talk to your doctor before deciding to stop. Stopping isn’t a failure.

Are minoxidil tablets a good idea?

Some doctors prescribe very low-dose minoxidil tablets. This is not the same as the lotion: the tablet acts throughout the body, and was originally a medication for high blood pressure. In a trial involving 52 women, 1 mg per day produced results comparable to 5% lotion, but caused more body hair (27% of women, compared with 4%). A large study of 1,404 patients found no life-threatening effects, but it did not have a comparison group. I’ve never taken it. Never take minoxidil tablets without a prescription and medical supervision, and never swallow the lotion intended for the scalp.

What if minoxidil doesn’t work?

Other options exist; discuss them with a dermatologist. For finasteride in women, the Cochrane review found no benefit. For laser combs, it noted an increase in hair count, but the evidence was low quality. Cyproterone acetate (Androcur) is sometimes suggested: the ANSM warns of a risk of meningioma, a tumor of the membranes surrounding the brain, and strictly regulates its use. I refused it myself.

There’s also everything that doesn’t treat the condition but makes a difference in everyday life: the hair-thickening powder for a widening part, the hair topper for restoring volume, and a wig when hair loss is more extensive. To choose, read wig or hair topper: which should you choose? If you want to understand what’s behind your hair loss, here’s the main causes of hair loss.

Your quick questions

Do you need a prescription for minoxidil?

No, 2% minoxidil is available over the counter at pharmacies. I still recommend having a dermatologist confirm your diagnosis before you start.

Does minoxidil make your hair greasy?

It can happen: for me, the lotion left my roots looking dull, and my hair felt dirty.

Do you have to take minoxidil for life?

The effects last as long as you use it. If you stop, things return to how they were within a few months.

Is minoxidil dangerous?

When used as directed on the label, it mainly causes minor skin reactions. But it is a medication, with contraindications and precautions to follow, especially around babies, children, and pets.

What is the best minoxidil for women?

In France, the recommended strength for women is 2%. The active ingredient is the same from one brand to another; the choice mainly depends on how well your scalp tolerates it.

My experience in a nutshell

I used 2% and then 5% minoxidil for almost 10 years to treat my androgenetic alopecia, with no noticeable results. I stopped when we decided to have a baby. At 30, I wore my first hair topper, and it changed my life. My full story is in my personal story about minoxidil.

Sources

This guide provides general information and is not a substitute for a medical consultation.
